Colleen Fleming is a Board Certified Psychiatric Mental Health Nurse Practitioner with over 9 years of experience. Colleen works with adults 18 and up experiencing anxiety, depression, PTSD, Bipolar Disorder, and Borderline Personality Disorder.. She has experience providing psychiatric intakes and evaluations, diagnosing, and prescribing medications for patients with both mental health issues and substance abuse issues. She has worked hand in hand with Primary Care Providers in a rural, underserved, community. More recently Colleen has worked in her own private practice setting. She has extensive experience working in Community Outpatient setting and enjoys helping people be their healthiest and best selves.

Colleen received her Bachelor or Arts in Anthropology from University of Delaware, her Bachelor of Science in Nursing from Oregon Health and Science University, her Masters in Nursing from University of Cincinnati, and a Post Masters Certificate in Nursing from Eastern Kentucky University.

Colleen enjoys learning new things, being outdoors, cross country skiing, gardening, traveling, and taking long walks with her dog.

Giving back to communities in which she has lived has been a theme throughout Colleen’s career.

Girls on the Run Volunteer assistant coach, Spring 2013 Sharon Elementary School Nurse Council

Representative 2009-2011 Representing the Nurse Partnership at Multnomah Education Service District.

Volunteer at Day treatment Program Veteran’s Affairs, Portland, Oregon January-July 2001. VA hospital outpatient mental health program. Assisted leading weekly music group on a volunteer basis.

Volunteer at the Free Clinic of Cleveland, OH 1995-1997. Mental health intake interviewer at inner city, walk-in free clinic. Worked with a diverse population of clients conducting initial psychiatric interviews and assessments, and mental health crisis intervention.
